Toyota recalls 1.3 million vehicles as it regains #1 position as car manufacturer

Toyota recalls 1.3 million vehicles, mostly in America for technical glitches involving airbag and wipers. This partiallly clouds its feat of reclaiming the # l position as the worlds car manufacturer from GM having sold 9.748 million vehicles.
In October, Toyota recalled 7 million vehicles. It was the single biggest recall, after Ford recall of in l996.
Toyota has been hounded by a large number of recalls/defects. In 2009 and 2010 Toyota faced 12 million recalls over accelerator pedal problems.
Why would such quality problems face a car manufacturer noted for JIT, Toyota Production System and other quality lessons in manufacturing?
What happenned? Are its workers and supervisors now complacent?
